The physician's medical certification stated that he had attended the deceased from Nov 25 1942 to Apr 17 1948, that he had last seen him alive on Mar 27 1928, and that the date of death was Feb 17 1948. The certification was dated Apr 17 1948, and date of burial was stated as Apr 19 1948. The date of death was clearly intended to be Apr 17 1948, the same date inscribed on the headstone. The birth date and age at death recorded on the death record also calculate to the same Apr 17 1948 death date. The handwritten entries made on this form were the basis for the typed entries subsequently made for the death record on file at Harrison County, which reflects the same conflicting information and incorrect death date.
