Event Notes Note (1) "Davis - The Settlers of Salem, West Virginia," p. 60, incorrectly reports place of death as Grafton in Taylor County. But a newspaper account states that Thomas Ford, a resident of Grafton, died instantly at Tunnel #6 east of West Union when the train he was conducting collided with another train. One of the three physicians who responded to the accident scene was E. B. Fittro of Salem, who incorrectly recorded the death in Harrison County, with place of death entered as "near Salem," cause of death "killed by train," further described as "killed instantly." Burial location was given only as Grafton, his age was incorrectly stated as 42y 4m 5d, and his occupation was incorrectly given as brakeman (which was his occupation in the 1900 Census), rather than conductor as reported in the newspaper account of his death. The death was reported by the physician on Sep 3 1904, nearly five months after it occurred, which may account for some of the errors. Note (2) Birth date given in death record as Jul 22 1866. But "Davis - The Settlers of Salem, West Virginia," p. 101, reports it as Jul 22 1867, which is supported by birth date July 1867 in the 1900 Census and age 3 in the 1880 Census.
