But the WV Births Index shows his birth date as Mar 24 1903. His birth record shows his middle name as Leo, but his draft registration indicaes Lee. Note (3) There is a marker for her at the Masonic cemetery at Crystal Lake, West Union. Inscription: Carrie Underwood Karwash 1905 - (blank). But Findagrave reports her burial in Pennsylvania.
