This is a delayed birth record recorded on May 17 1963 on authority of an "affidavit signed by neighbor, drivers license & Social Security Application," dated July 16, 1966. The record was filed on Nov 3 1966. (Note: Accuracy of this birth date is questionable. Age 5 in the 1920 Census and age 13 in the 1930 Census would put birth between about 1914 and 1917.
